If you sat out for one or more terms, briefly outline your activities during your time off (Classes or Trainings completed, Jobs Held, Volunteer Experiences, Online Certifications completed, etc.). Do you feel that these experiences will contribute to your academic success if you are allowed to return to school? If so, how? (300 words max)
Many students are unsuccessful when they return to school after suspension, because the underlying circumstances have not yet been resolved. Therefore, they return to school but are unable to focus on their school work.
If your situation has been fully resolved, please explain how it was resolved and why you are certain it will no longer be an issue. If the situation is not fully resolved, how will you manage it while completing your academic studies? (300 words max)

You are required to provide documentation that reflects the semester for each of the circumstances above. The goal is to demonstrate that your academic progress was impacted by circumstances beyond your control. The committee will review your information to establish the truth of your claims, assess the severity of the impact, and evaluate the timeframe of the incident as compared to your academic downturn. Examples of the types of documentation that may be helpful in building your case include, but are not limited to, the following:
