2. Provide the intended year and semester of enrollment.
3. Review the course offerings at the outside institution and identify the course that you intend to complete.  In the first and second columns, list the Course ID, Course Title and Credit Hours for the outside course (e.g. - MATH 101 General Math, 3 cr).
4. Identify the Benedict course for which this class will be substituted.  List the Course ID, Course Title, and Credit Hours of the Benedict course in the third and fourth columns (e.g. - MATH 132 General College Math, 3 cr).
5. Indicate your agreement with each of the statements listed at the bottom of the form.

1. I understand that I am solely responsible for completing the admissions, registration, and payment processes of the transfer institution.  Upon completion of the course, I am responsible for submitting an official transcript from the transfer institution to Benedict's Registrar's Office.  
2. I understand that I must be in good financial standing with Benedict College in order to be approved to enroll as a transient student elsewhere.
3. I understand that I must have a minimum cumulative GPA of 2.0 in order for my request to be approved.
4. I understand that I may not repeat a course at another institution to remove a grade of D or F earned in the Benedict equivalent.
5. I understand that I must earn a grade of C or better in order for course credits to be transferred back to Benedict College. 
6. I understand that upon receipt of an official transcript demonstrating an acceptable grade, credit hours will be applied to my Benedict transcript; however, the transferred course will not impact my Benedict GPA.
7. I understand that exceptions to this policy may only be approved by the School Dean and Vice President for Academic Affairs, or the President.
